About BLKWell and these Terms

These Terms of Service govern your use of BLKWell, a health-navigation platform operated by Culbreath Holdings LLC. Culbreath Holdings LLC is the legal business entity responsible for the BLKWell platform and its services.

In these Terms, "BLKWell," "we," "us," and "our" refer to Culbreath Holdings LLC, operating the BLKWell platform.

What BLKWell is

BLKWell is a free educational tool that helps Black Americans living with disabilities or chronic health conditions navigate the healthcare and benefits system. It is not a healthcare provider, insurance company, legal service, or government agency.

The information in this app is for educational purposes only. Only official government agencies can make eligibility determinations for programs like SSDI, SSI, or Medicaid.

Using BLKWell

• You must be 13 years of age or older to create an account.
• You agree not to submit false, harmful, or misleading provider reviews.
• You agree not to use this app for any unlawful purpose.
• Provider ratings reflect community experience, not clinical quality assessments. They are not professional medical recommendations.

SMS Messaging Terms

If you choose to receive SMS messages from BLKWell, you agree that BLKWell may send customer-care and health-navigation text messages related to services or assistance you have requested.

Messages may include appointment reminders and confirmations, responses to requests for assistance, referral and care-navigation updates, and other customer-care communications related to your interactions with BLKWell.

Message frequency varies based on your requests and interactions with BLKWell. Message and data rates may apply.

You may opt out of SMS messages at any time by replying STOP. For assistance, reply HELP.

SMS consent is optional and is not a condition of receiving BLKWell services.

BLKWell does not use the SMS program for unsolicited promotional messages.

Community reviews

Reviews submitted by community members reflect personal experience only. BLKWell moderates reviews to remove harmful or inappropriate content, but we do not verify every submission. Reviews are not endorsements.

Limitation of liability

BLKWell and its operators are not liable for any decisions made based on information in this app. Always verify important information with an official agency or licensed professional.

Changes to these terms
