What we collect

When you use BLKWell, we may collect the following information:

Name — so we can address you personally
Email address — to create your account or send you updates you signed up for
City — to help surface relevant local providers
Provider ratings and reviews — your community experience ratings submitted on the Provider Finder
Feedback messages — comments you submit through the feedback button
Appointment prep inputs — the appointment type and concerns you enter to generate a prep sheet. These are used only to create your sheet and are not stored after you leave the page.

What we NEVER collect

We are designed to stay outside the scope of health data regulation. We will never ask for or store:

Diagnoses or medical conditions
Medications or dosages
Medical records or test results
Social Security numbers (SSNs)
Insurance member IDs or policy numbers
Payment card information

We never sell your data. Period.

How we use what we collect

To run and improve the BLKWell app
To send you updates if you joined our mailing list (you can unsubscribe anytime)
To display community provider ratings to other users
To respond to your feedback

We do not share your personal information with third parties for marketing purposes.

SMS Messaging and Mobile Information

If you voluntarily provide your mobile phone number and affirmatively consent to receive text messages from BLKWell, BLKWell may use your mobile number to send customer-care and health-navigation communications related to services or assistance you have requested.

Message frequency varies based on your requests and interactions with BLKWell. Message and data rates may apply.

You may opt out of SMS messages at any time by replying STOP. You may reply HELP for assistance.

BLKWell does not sell, rent, or share mobile phone numbers or SMS consent information with third parties for their own marketing or promotional purposes. SMS opt-in information will not be used or disclosed for third-party marketing.

Providing SMS consent is optional and is not a condition of receiving BLKWell services.

BLKWell will maintain records of SMS consent for compliance and operational purposes.

Your rights and how to request deletion

You have the right to access, correct, or delete any personal information we hold about you. To request deletion of your account and all associated data, email us at:
